New Patient Information

Welcome to Trident Heart and Vascular. Thank you for choosing us to take better care of your health. Please help us serve you better by providing the necessary information below.

• Please print the new patient registration forms, complete them at home, and bring them to your next scheduled in-person visit. 

• Please print the "medical record release form", complete it at home, and return it to our office prior to your next visit if possible.   

• New patients need to arrive at least 20 minutes prior to their scheduled appointment time to complete the check-in process. 

• We require the following for all in-person visits:

1. Driver's license or Valid Government issued ID 

2. Insurance or Medicare cards, and/or any required insurance referral or authorization, or Medicaid paperwork. 

3. Please bring all of your medications and/or recent test results with you. Also bring any previous health record from your previous doctor.​​

• All imaging and other test results will be discussed during a follow up visit. The follow up appointment will be made at the time of scheduling the procedure.
